A Chronic Disease Management (CDM) Plan is a Medicare program that helps people with long-term conditions access allied health services, including Speech Pathology and Occupational Therapy.
Your GP creates a CDM Plan if you have a condition that lasts six months or longer, such as Autism, developmental delays, or speech and language disorders.
The plan provides Medicare rebates for up to five allied health sessions per year. These sessions can include Speech Pathology, Occupational Therapy, or other allied health services.
Therapy for communication challenges, motor skills, sensory needs, or daily living activities.
You’ll receive a rebate for each session, though a gap fee will still apply.
Talk to your GP about your needs.
Your GP will create a plan and refer you to a therapist.
Book your sessions with the referral to begin accessing support.
If you think you or your child could benefit from a CDM Plan, speak with your GP or contact us at Eat Talk Play for more information.
Many private health insurance policies include cover for Speech Pathology and Occupational Therapy services, typically under “Extras” or “Options” policies.
To find out what your policy covers and how to claim, we recommend contacting your private health fund directly for detailed information about your benefits and their claiming process.
Please note, Eat Talk Play does not currently offer HICAPS for on-the-spot claims. However, we are happy to provide invoices so you can claim directly through your health fund.
